Request: Banner

I'm looking for a very simple banner.

Size: Small (about 250x100)
Content:

Must have the following text: "white blocks." and "by yihwan kim" right undearneath it in Georgia like font. white blocks should be slightly larger than yihwan kim. black font please.

Must contain this image: http://webuser.bus.umich.edu/organiz...tethoscope.jpg
Somewhere off to the side. If you could whitewash it so that the background is not so dark, that'd be great. Also, if you could sort of blur it to get a surreal effect, that'd be nice too.

Background should be white.

i'm still not used to how the bp system works, so i'll pay what ever you ask.
