First, go to File>New> (400x200) and press "ok"
and type a text you would like. Here, I used "HARU"
make the text Impact (You can also use Arial, but Impact seems to work better, it's your choice), 125px, color #336EA7, and text mode to "Crisp":
Layer>Rasterize>Layer
Image>Rotate Canvas> 90degrees CCW:
Now do Filter>Stylize>Wind and apply like the picture below,
Apply Ctrl+F twice to repeat the process 2 more times:
Image>Rotate Canvas> 90degrees CW:
right-click on the layer and click on "Duplicate Layer":
While the "(Name) copy" is highlighted, do Filter>Blur>Motion Blur and apply like the picture below:
Now click on the Eraser Tool and make the size 65, mode to Brush, and opacity 100% and erase the place where I have colored in red:
Now apply Layer>Merge Down
and then apply Filter>Stylize>Find Edges
And the final product should look like:
